Saint Mary's Healthy Decisions for Your Heart Program

Saint Mary's Health Plans is pleased to offer its members the Healthy Decisions for Your Heart Program. This telephone and mail-based program provides a team of experienced registered nurses, registered dietitians and respiratory therapists to provide information and support to members and their practitioners in managing members' coronary heart disease and congestive heart failure. This free program is available to Saint Mary's HealthFirst and Health Choice members.

Benefits of the program include:

1. A health professional to contact you at your convenience to answer questions and provide information to help you carry out the plan of care you and your physician have developed.

2. An cardiac workbook with health information, places to keep your medical records up to date, and stickers to remind you when your health screenings are due.

3. Mailed health information and reminders for screenings and flu shots.

4. A toll-free number to call for information about your coronary heart disease or congestive heart failure, including questions about nutrition, activities, medications, recommended health screenings and resources in the community.

5. An in-home monitoring program for members with congestive heart failure and additional risk factors.
