Conducted in a relaxing environment, this comprehensive physical exam makes it convenient to obtain a health checkup. Test results from this two-hour physical are sent to your Primary Care Physician for follow-up to ensure your continuum of care.
Health screenings include: BMI (Body Mass Index); Chest X-ray, EKG, hearing screening, height and weight, metabolic syndrome assessment, pulmonary function spirometry, vision screening and vital tests.
Lab tests include: Complete Blood Count (CBC), comprehensive metabolic panel, Free T4 (thyroid test), hemoccult cards, lipid profile, magnesium, TSH (Thyroid Stimulating Hormones), uric acid, urinalysis and Vitamin D 25-Hydroxy.
This test, valued at $600, is provided to SMHP members for a $100 copay. It is also suggested that you consult your formal Summary of Benefits to determine member responsibility.
