Pharmacy Network

Dental Network

Family Planning Network
Saint Mary's Health Plans is contracted with Unified Life Insurance Company to provide family planning benefit coverage. If an employer has purchased supplemental contraceptive and/or sterilization coverage for a Saint Mary's Health Plans medical plan, member must call 800.342.2641 to obtain plan and benefit information.
